22-24 мая 2015 года в столице нашей страны, Астане, прошла II Научно-практической конференции: «Экстракорпоральная мембранная оксигенация (ЭКМО) в Казахстане: актуальные вопросы и пути развития».
Экстракорпоральная мембранная оксигенация (ЭКМО) – процедура продленного искусственного кровообращения и насыщения крови кислородом (оксигенация) вне организма (экстракорпорально), используемая у пациентов с остро развившейся и потенциально обратимой дыхательной или сердечной недостаточностью, лечение которых не поддаётся максимальной стандартной терапии. Для перекачивания крови (искусственного кровообращения) применяются машины, схожие с аппаратом «сердце-лёгкие», используемом в хирургии сердца. При этом соединение системы трубок аппарата с организмом человека происходит путем их введения в крупные артерии и вены, или непосредственно в камеры сердца.
Конференция началась с вручения наград за активное участие и развитие ЭКМО в Казахстане специалистам АО "Национальный Научный Центр Материнства и Детства", совместными усилиями с которыми ЭКМО стало «мостом» к восстановлению здоровья и рождению детей!
Большой и дружный коллектив операционного отделения с лабораторией вспомогательного кровообращения и отделений анестезиологии, реанимации и интенсивной терапии (взрослое, детское) АО "Национальный научный кардиохирургический центр" также были награждены специальным кубком "ЭКМО Казахстана".
В программу Конференции вошли выступления родоначальников и безусловных лидеров ЭКМО службы Франции: профессор Pascal Leprince, Gulliame Lebreton и Nicola Brėchot из Institut de Cardiologie Groupe Hospitalier Universitaire Pitié-Salpêtrière. Данная клиника является самым крупным центром мира, выполняющим до 500 процедур ЭКМО ежегодно. Важно отметить, что команда из этого центра эффективно занимается не только экстракорпоральной мембранной оксигенацией, но и имеет колоссальный опыт в проведении любой механической циркуляторной поддержки. Профессор Pascal Leprince – кардиохирург, со своей командой выполнили более 200 имплантаций полностью искусственного сердца SynCardia и более 2000 пересадок сердца. Его коллега Gulliame Lebreton, также кардиохирург, блестяще организовал программу транспортного ЭКМО на территории Карибских островов, в нескольких тысячах километров от Парижа! Сегодня, работая в клинике Pitié-Salpêtrière, транспортное ЭКМО получило новое развитие и связало эти отдаленные территории Франции. Nicola Brėchot – доктор интенсивист (реаниматолог) обладает бесценным опытом выхаживания ЭКМО пациентов, в отделении интенсивной терапии Institut de Cardiologie, что отражено в многочисленных публикациях в титулованных журналах.
Именно в Institut de Cardiologie Groupe Hospitalier Universitaire Pitié-Salpêtrière команда кардиохирургического центра прошла обучение в течение 3 модулей и получила международный ЭКМО диплом.
Прошедшая конференция в Национальном научном кардиохирургическом центре имела практическую направленность. В ней приняли участие врачи и специалисты сестринского дела из городов, как: Астана, Алматы, Павлодар, Костанай, Семипалатинск, Актобе, Кызылорда, Петропавловск, Тараз, Караганда, Шымкент, Уральск, Актау, Кокшетау, а также из областей нашей страны. Из них в 9-ти крупных клиниках установлено соответствующее оборудование для проведения ЭКМО. Коллеги из Парижа поделились своим опытом экстракорпорального поддержания жизнедеятельности во всех аспектах: от определения показаний к проведению процедуры, до мероприятий по выхаживанию, реабилитации пациентов и отдаленным результатам лечения. В свою очередь врачи и специалисты сестринского дела ННКЦ, выступая перед слушателями конференции, рассказали об особенностях менеджмента пациентов на ЭКМО в Казахстане.
Во все дни проходили тренинги по работе с аппаратами ЭКМО, когда возможно было ознакомиться с техническими характеристиками всех устройств,
В завершающий день (не смотря на воскресенье) все гости в «командном» режиме смогли выполнить канюляцию (установку канюль) на манекене, запустить систему ЭКМО в работу и отработать различные ситуации, с которыми возможно столкнуться в реальной клинической практике.
Экстракорпоральная мембранная оксигенация в Казахстане, как и во всем мире, вышла за рамки оказания помощи только больным с сердечными заболеваниями. Поэтому в ходе конференции был проведён расширенный круглый стол в присутствие гостей из Pitié-Salpêtrière, ведущих специалистов в области кардиохирургии регионов Казахстана, представителя министерства социального развития и здравоохранения, санитарной авиации, главного пульмонолога Республики, представителей центра материнства и детства. Были обсуждены текущие вопросы организации ЭКМО службы в нашей стране, согласована программа поэтапной регионализации службы и создание Республиканского координационного и обучающего центра на базе ННКЦ.
АО «Национальный научный кардиохирургический центр» занимается ЭКМО с начала своей деятельности. Количество пациентов, которым ЭКМО дало «мост к выздоровлению», увеличивается каждый год. В АО «Национальный научный кардиохирургический центр» данный метод доступен для пациентов всех возрастов – от новорожденных до пожилых.
С октября 2011 года по настоящее время в АО «Национальный научный кардиохирургический центр» ЭКМО было установлено 150 больным, из них: 37- детям (0- 17 лет) и 113 - взрослым старше 18 лет.
Благодаря лечению самой сложной категории пациентов АО «ННКЦ» вошел во всемирный регистр ELSO (Extracorporeal Life Support Organization). Эта организация – международный консорциум специалистов медиков и учёных, посвятивших себя разработке и развитию новейших технологий поддержания жизнеобеспечения (Life Support). На основе данных, накапливаемых в регистре, ELSO поддерживает клинические исследования, публикует практические руководства.
Надеемся, что, в скором времени ЭКМО будет развито в нашей стране не меньше, чем во Франции и на мировой карте ЭКМО географически «зажгутся» регионы Казахстана.
The II Scientific-practical conference «Extracorporeal membrane oxygenation (ECMO) in Kazakhstan: current issues and development trends» was hold in the capital of our country – Astana – on 22-24 May 2015.
Extracorporeal membrane oxygenation (ECMO) is a technique of extended artificial circulation and oxygen saturation (oxygenation) outside of a body (extracorporeal), which is used for patients with acutely developed and potentially reversible breathing or heart failure that cannot be treated with a standard therapy. For blood pumping (artificial blood circulation) the equipment similar to the heart-lung machine, which is applied in cardiac surgery, is used. The connection of the tube system of the equipment to the body is initiated by placing the tubes in large arteries and veins, or directly in cardiac chambers.
The conference commenced with presenting the awards for active participation and contribution to the development of ECMO in Kazakhstan to the specialists of JSC “National Research Center for Maternal and Child Health”; whose collaborative effort has led the ECMO practice to become “the bridge” to recovery and child birth!
The friendly staff of the operational unit along with the laboratory of circulatory support and anesthesiology and intensive care (adult, pediatric) departments of JSC "National Research Center for Cardiac Surgery" were also presented with a special award - "ECMO in Kazakhstan."
The programme of the Conference contained the speeches of the founders and absolute leaders of ECMO of France: Prof. Pascal Leprince, Gulliame Lebreton and Nicola Brėchot from Institut de Cardiologie Groupe Hospitalier Universitaire Pitié-Salpêtrière. This clinic is the world’s biggest center, which performs up to 500 ECMO procedures every year. It should be noted that the team of this center not only effectively deals with extracorporeal membrane oxygenation, but also has a tremendous experience in performing any mechanical circulatory assistance. Prof. Pascal Leprince, a cardiac surgeon, and his team have completed more than 200 implantations of total artificial heart SynCardia and more than 2,000 heart transplants. His colleague Gulliame Lebreton, who is also a cardiac surgeon, has brilliantly organized the program of transport ECMO on the territory of the Caribbean Islands, several thousand kilometers away from Paris! Today, being implemented in Pitié-Salpêtrière Hospital, transport ECMO has received a new way of development and has connected these remote areas of France. Nicola Brėchot - intensivist (expert in resuscitation) has priceless experience of nursing ECMO patients in an intensive care unit of Institut de Cardiologie, which is reflected in numerous publications in the well-known journals.
In particular, the team of National Research Center for Cardiac Surgery has undergone a 3-module and received an international diploma in ECMO from the Institut de Cardiologie Groupe Hospitalier Universitaire Pitié-Salpêtrière.
The conference at the National Research Center for Cardiac Surgery had a practical orientation. It was attended by the doctors and specialists of nursing from cities, such as: Astana, Almaty, Pavlodar, Kostanay, Semey, Aktobe, Kyzylorda, Petropavlovsk, Taraz, Karaganda, Shymkent, Uralsk, Aktau, Kokshetau and also from other regions of our country. In 9 big hospitals, the appropriate equipment for ECMO has already been assembled. The colleagues from Paris shared their experiences of extracorporeal life assistance in various aspects: from defining the indications for the procedure to nursing activities, patients' rehabilitation and long-term outcomes. In turn, the doctors and the specialists of nursing care of the National Research Center for Cardiac Surgery talked about the peculiaritites of ECMO patient management in Kazakhstan.
The trainings about working on the ECMO equipment were provided on each day of the Conference in order to allow the participants to get familiar with the technical characteristics of all devices.
During the last day (despite Sunday), all participants had an opportunity to complete the сannulation (the installation of cannulas) on a mannequin, run an ECMO system and work out different situations which can happen in real clinical practice.
Extracorporeal membrane oxygenation in Kazakhstan, as well as in the whole world, has gone beyond the limits of helping only patients with cardiac diseases. Therefore, during the conference, an extended round table was hold with the presence of guests from the Pitié-Salpêtrière, the leading experts in the field of cardiac surgery from various Kazakhstan regions, the representative of Ministry of Health and Social Development, air ambulance, the chief pulmonologist of the Republic of Kazakhstan, the representatives of the Center of Maternal and Child health. The current issues of organizing the ECMO service in our country were discussed; the program of phased regionalization of the service and the establishment of the National Coordination and Training center on the premises of National Research Center for Cardiac Surgery were approved.
JSC “National Research Center for Cardiac Surgery” has been developing ECMO practice since its establishment. The number of patients increases every year. This technique is available at JSC “National Research Center for Cardiac Surgery” for patients of all ages – from infants to elderly patients.
From October 2011 till present, at JSC “National Research Center for Cardiac Surgery” ECMO was performed in 150 patients, of whom: 37 children (0-17 years old) and 113 adults who are older than 18 years old.
Owing to the treatment of the most complicated category of patients, JSC “National Research Center for Cardiac Surgery” was included in the world ELSO register (Extracorporeal Life Support Organization). This organization is an international consortium of the medical specialists and scientists who devoted themselves to the development and improvement of the latest life support technologies (Life Support). Based on the data, which is accumulated in the register, ELSO supports clinical researches, publishes practical guides.
We hope that in the near future, ECMO will be developed in our country on the same level as it is in France, and Kazakhstan will appear on the world map of ECMO.
